By signing this form, I acknowledge and agree as follows:

1. Representation of Health and Fitness
I declare that I (or the minor participant) am physically fit, have no known medical conditions that would prevent safe participation, and have not been advised otherwise by a qualified medical professional. I understand it is my responsibility to consult a physician regarding any concerns about my ability to participate.

2. Assumption of Risk
I understand that participation in Touch Rugby involves inherent risks, including but not limited to falls, collisions, contact with other participants, weather-related conditions, and use of equipment or facilities. I voluntarily assume all risks, both known and unknown, associated with participation in this Event.

3. Release and Waiver of Liability
I hereby release, waive, and forever discharge Toronto Touch Rugby, its directors, officers, employees, volunteers, sponsors, event organizers, venue operators (including MLSE and Lamport Stadium), and their respective affiliates, agents, and representatives (collectively, the “Released Parties”) from any and all liability, claims, demands, actions, or causes of action arising from or related to any loss, damage, injury, or death sustained during or as a result of participation in this Event, whether caused by negligence or otherwise.

4. Indemnification
I agree to indemnify and hold harmless the Released Parties from any claims, demands, damages, losses, or expenses (including legal fees) arising from my (or the minor’s) participation in this Event.

5. Acknowledgment of COVID-19 and Communicable Disease Risks
I understand that participation may expose me (or the minor) to contagious illnesses, including COVID-19. I voluntarily assume these risks and agree that the Released Parties are not responsible for any illness, injury, or loss resulting from exposure.

6. Agreement Not to Sue
I agree not to commence or participate in any claim or legal proceeding against the Released Parties related to participation in this Event.

7. Waiver of Insurers’ Subrogation Rights
I waive any right my insurers may have to make a claim against the Released Parties for payments made on my behalf related to injury, loss, or damage incurred during the Event.

8. Medical Authorization
In the event of an injury or medical emergency, I authorize Toronto Touch Rugby and its representatives to secure medical treatment deemed necessary, and I agree to assume all associated costs.

9. Media and Publicity Release
I grant Toronto Touch Rugby, MLSE, and their affiliates the irrevocable right to use my (or the minor’s) name, photograph, likeness, voice, and image in any media, for promotional or informational purposes, without compensation or further notice.

10. Property Damage Responsibility
I agree to reimburse the Released Parties for any property damage caused by my (or the minor’s) actions.

11. Behavioral Expectations
I agree to conduct myself (or ensure the minor conducts themselves) in a respectful manner toward Lamport Stadium staff, officials, and participants. Misconduct, vandalism, or inappropriate behavior may result in removal from the league without refund. (See Player Code of Conduct.)

12. Force Majeure
I acknowledge that league play may be canceled, postponed, or interrupted due to circumstances beyond the control of the Released Parties (e.g., extreme weather, natural disasters, pandemics, or government orders), and that no refunds or compensation will be provided for such disruptions.

13. Legal Jurisdiction and Binding Effect
This waiver shall be governed by and interpreted in accordance with the laws of Ontario. It is binding upon me, my heirs, executors, administrators, successors, and assigns.

14. Severability
If any provision of this waiver is deemed invalid or unenforceable, the remaining provisions shall continue in full force and effect.
